The contact form collects the information you enter, including your name, email, optional phone number, DOT mode, and message. It is used to respond to your inquiry and coordinate service requests. Do not submit medical records or identification numbers through this public form.
Third-party services
Form submissions are processed by Formspree. The popup chatbot is provided through Zapier. Their handling of technical data, cookies, and submitted content is governed by their own policies. External resource and map links lead to third-party sites.
Clinical records
This website notice does not replace clinical consent, recordkeeping, or required DOT reporting disclosures. Those are addressed through the service process. Do not use chatbot conversations as a clinical record submission channel.
Questions and requests
Use the contact page to request correction or deletion of an inquiry or ask about data handling. Legally required records and DOT reports may have retention obligations. Avoid sensitive information when making a privacy request.
