Bring the information that helps clarify your case
- Your photo identification and current contact information.
- Your employer or C/TPA contact and designated employer representative, if available.
- The DOT mode and violation information from your testing documents.
- Relevant prior education or treatment records when requested through an appropriate channel.
The evaluation considers your substance-use history and needs. The SAP determines education and/or treatment recommendations individually; recommendations are not a package you select in advance.
Two evaluations, distinct purposes
The initial evaluation leads to recommendations. A follow-up evaluation occurs after your documented participation and allows the SAP to assess compliance. Completing a class alone does not automatically complete the SAP process.
$499 SAP package
The package includes the initial evaluation, written recommendations, education/treatment referrals, follow-up evaluation after documented compliance, applicable SAP reporting, and a follow-up testing plan. Referrals are included; the services provided by the referred organizations are billed separately.
