This site explains DOT SAP and return-to-duty processes. It does not provide an individual diagnosis, legal advice, employment approval, or a guarantee of license restoration. Individual recommendations follow an evaluation.
Service requests and pricing
Sending a form does not confirm an appointment or create a clinical relationship. Confirm availability, appointment format, fees, and service terms directly. The advertised $499 SAP package excludes third-party education, treatment, and testing costs. There is no guaranteed completion timeline.
Employer decisions
The SAP determines compliance with recommendations. The employer handles required testing and decisions about resuming safety-sensitive work. Agency-specific rules may add requirements.
Third-party resources
External sites operate under their own terms. Official DOT and FMCSA resources should be consulted for current regulatory requirements. This independent service is not a government agency.
